Form C Semi-Annual Report

As a contract supplier, you are required under the terms of your contract with the Centers for Medicare & Medicaid Services (CMS) to complete the Form C semi-annual report no later than 10 business days after each of the following dates: June 30 and December 31, throughout the entire contract performance period. The information provided in the report is used to update the Supplier Directory on the Medicare website with the products you plan to provide during the next six-month period.

The first Form C report is due no later than Friday, January 15, 2021, for every competitive bidding area and product category combination included on your contract. To complete and submit your report, go to Connexion, the Durable Medical Equipment, Prosthetics, Orthotics, and Supplies (DMEPOS) Competitive Bidding Program's (CBP’s) secure portal, and select Form C under the Contract Maintenance tab at the top of the page or on your My To-Do List.

Contract Change Requests

You can also request changes to your contract, such as adding or removing a location or transferring a contract, in Connexion. Please visit Contract Update Requests on the Competitive Bidding Implementation Contractor (CBIC) website for more information.
