Time to Submit Your Form C Semi-Annual Report

Your semi-annual Form C report for the first reporting period is now available on your My To-Do List in Connexion, the Durable Medical Equipment, Prosthetics, Orthotics, and Supplies (DMEPOS) Competitive Bidding Program’s (CBP) secure portal. Form C is used to update the Supplier Directory on the Medicare website with the products you plan to provide during the next six-month period.

As stated in your DMEPOS Competitive Bidding contract, the form is due no later than ten business days after each of the following dates: June 30 and December 31, throughout the entire contract performance period. You must complete Form C in Connexion no later than January 14, 2022, at 11:59:59 p.m. prevailing Eastern Time, for every competitive bidding area (CBA) and product category combination included in your contract.
