Time to Submit Your Form C Semi-Annual Report

Your Form C semi-annual report is now available on your My To-Do List in Connexion, the Durable Medical Equipment, Prosthetics, Orthotics, and Supplies (DMEPOS) Competitive Bidding Program’s (CBP’s) secure portal. Form C is used to update the Supplier Directory on the Medicare website with the products you plan to provide during the next six-month period.

As stated in your DMEPOS CBP Round 2021 Contract, Form C is due no later than 10 business days after each of the following dates: January 1 and July 1, throughout the entire contract performance period. You must complete Form C in Connexion no later than January 17, 2023, at 11:59:59 p.m. prevailing Eastern Time, for every competitive bidding area and product category combination included in your contract.

Prior to submitting the required semi-annual report, review the Supplier Directory on the Medicare website to determine whether your information is current. Contract supplier information can be updated in the Provider Enrollment, Chain, and Ownership System (PECOS), and must remain current so claims can be properly processed and beneficiaries can contact you to obtain items and services. You must submit updates in PECOS so that any outdated information can be replaced with current information in the Medicare Supplier Directory. The National Provider Enrollment Eastern Region DMEPOS contractor (NPEAST) Change of Information Guide and the National Provider Enrollment Western Region DMEPOS contractor (NPWEST) Change of Information Guide are available to assist you.
