Form C reports for the second quarter of 2016 are due no later than Sunday, April 10, 2016.

If you have not yet submitted your Form C report for the second quarter of 2016, go to the Competitive Bidding Implementation Contractor (CBIC) website at www.dmecompetitivebid.com and select the applicable round(s) - Round 1 Recompete or Round 2 & National Mail-Order - from the menu on the left side of the homepage. Then select Form C under the Top Links section. Detailed information about Form C is available on that page.

Failure to submit a Form C for all competitive bidding area (CBA)/product category combinations in your contract may be considered a breach of contract and result in termination. However, since the Round 2 and national mail-order contract period ends on June 30, 2016, we are not enforcing the submission of Form C for the second quarter of 2016. Please note this requirement is optional for only Round 2 and/or national mail-order contract suppliers since this will be the last update before the end of the Round 2 and national mail-order contracts. If you have a Round 1 Recompete contract, you must complete Form C for that contract for the second quarter.
